You can submit this using IRS Form 4070. Alternatively, a signed statement listing your name, address, SSN, employer’s details, the period covered, and the total tips received is acceptable. Your employer needs this data to calculate the correct FICA and income tax withholding. If your regular wages cannot cover these taxes, the uncollected amount will appear in Box 8 of your W-2, and you will be responsible for paying that balance when we file your annual return.
